Document everything, look for signs, once a few are present talk to your director and make the report, continue to monitor and look for signs.

You don’t need to be sure, if you send in a report do so if there seems to be a tangible reason to but you don’t have to wait for more signs or investigate on your own. Getting a report in sooner and being wrong is better than sending in a report too late or never sending one.
